The staff RN provides direct hands-on care for patients admitted to the inpatient units throughout the BPVAHCS. The RN will provide ADLs, family support and education, spiritual and psychological support, pain management and medication administration. The RN will have superior critical thinking skills to prevent or respond to emergency situations and must possess assertive, effective communication and teaching skills. The RN will possess the ethics and values that are consistent with the mission, values, priorities, goals, and resources of the Veterans Administration Healthcare System. RN will perform other duties under the direction of the nurse manager or designee as assigned. This position is not open to novice nurses.

Registered Nurse Med. Surg.

Surgical Med-Surg

Surgical Ward provides care to pre and post-surgical patients. Surgeries include general, urology, orthopedic, gastric bypass, plastics, vascular, ENT, and others. The RN will autonomously provide physical assessments and design a plan of care based on those observations and previous medical orders and history as well as admission assessments and primary nursing care. Responsibilities will include but are not limited to IV therapy, medication administration, nursing diagnosis, treatment plans, point of care testing, team leading, as well as patient and family education. RNs will assess, monitor, and treat the patients for pain; will administer blood and blood products as needed. RNs will provide for comfort and safety to all patients. The RN will occupy a leadership role with oversight of LPN and UAPs. RNs are required to participate in interdisciplinary rounds, unit base council, projects, and committees. RN will perform other duties under the direction of the nurse manager or designee as assigned.

The cardiac telemetry nurse functions under the direction and guidance of the nurse manager for the Cardiac Telemetry Unit in providing clinical patient-centered care on the off tours. The staff RN will need understanding of step-down cardiac units and cardiac infusions. The staff RN provides direct hands-on care for patients admitted to the inpatient units throughout the BPVAMC. General Medical/Oncology
The General Medicine/Oncology Staff RN provides direct patient care in support of ADLS, educational, spiritual and psychological support and pain management & medication administration. The RN will provide chemotherapy medications as prescribed within the facility policy and procedures. The RN will be responsible for admission/discharges, IV therapy, medication administration, nursing diagnosis, treatment plans, point of care testing, the RN will asses, monitor and treat the patient for pain, will administer blood and blood products as needed.

Registered Nurse Critical Care / Acute Care

Acute Care Registered Nurse

The Registered Nurse is assigned to the Nursing and Patient Care Service. The section, care line, or department description and patient population served is as defined in applicable Scope of Service. The RN practices in a complex patient care setting with minimal supervision. The nurse is accountable as assigned as a primary nurse for care of a defined group of patients on his/her shift or for an entire episode of care.

Critical Care Registered Nurse

The Registered Nurse will provide expert direct and individualized nursing care to acutely ill patients based on the application of scientific nursing principles. The RN will be an expert in the cardiac arena, providing cardiac and pulmonary support, frequent physical assessments, lab evaluations, blood and blood products, ongoing cardiac and pulmonary artery monitoring, pain control, EKG interpretation, and complex dressing changes.

The Critical Care Staff RN has the additional responsibility for the emotional and psychosocial health of the patient, family and significant others. The critical care staff RN supports the patients’ physical, spiritual, and psychological needs, in addition to providing family/significant others education and emotional support. The RN will possess the ethics and values that are consistent with the mission, values, priorities, goals, and resources of the Healthcare System.

The MICU Staff RN provides direct hands-on care for critically ill patients admitted to the medical critical care unit. The RN will provide expert direct and individualized nursing care to acutely ill patients based on the application of scientific nursing principles. The MICU Staff RN works in a highly technical and ongoing monitoring environment. The MICU patients are facing life-threatening problems with varying needs, which require extensive medication protocols and therapies. The MICU Staff RN has the additional responsibility for the emotional and psychosocial health of the patient, family and significant others. The MICU RN supports the patients’ physical, spiritual, and psychological needs, in addition to providing family/significant others education and emotional support. The MICU Staff RN has superior critical thinking skills to prevent or respond to emergency situations and must possess assertive, effective communication and teaching skills. The RN will possess the ethics and values that are consistent with the mission, values, priorities, goals, and resources of the Veterans Administration Healthcare System. The MICU Staff RN will perform other duties under the direction of the nurse manager or designee as assigned. This position is not open to novice nurses.

Preferred Experience:

Medical ICU

  • Associate Degree or Diploma with two (2) to three (3) years of successful critical care nursing practice – Required
  • Bachelor Degree in Nursing (BSN) with two (2) years of successful critical care nursing practice – Preferred
  • ICU, CCU, SICU, or like experience – Preferred
  • Current BLS and ACLS certification – Preferred
  • Demonstrate the ability to work effectively as a team member and leader – Preferred
  • Highly effective interpersonal, organizational skills, verbal, and written communication skills – Preferred
  • National Certification in Critical Care (CCRN) – Preferred

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

  • Nurse I Level I – An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II – An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor’s degree in a related field with no additional nursing practice/experience; OR a Bachelor’s of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III – An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor’s degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master’s degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II – A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor’s degree in a related field and approximately 2-3 year’s of nursing practice/experience; OR a Master’s degree in nursing or related field with a BSN and approximately 1-2 year’s of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III – Master’s degree in nursing or related field with BSN and approximately 2-3 year’s of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year’s of nursing practice/experience.

Physical Requirements:
Incumbent must possess the mental and physical ability to satisfactorily perform the duties of the proposed assignment. Physical requirements include visual acuity, keen hearing, clear distinctive speech, and manual dexterity. May involve potentially long periods of lifting, carrying, pulling, pushing, reaching above shoulder, walking, standing, sitting, kneeling, and repeated bending. Transferring patients and objects may be required. Safe patient handling equipment is provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive, sit and/or ride in GSA vehicles for long periods of time.

In-direct patient care work is mostly sedentary. Typically, the employee sits comfortably to complete the work. However, there may be some walking, standing, bending and carrying of light items such as patient records and supplies. In-direct care staff may be assigned to work in direct care areas as needed.

Nurse Assistant (CNA)

Major duties include, but are not limited to:

  • Assists with (bathing, shaving, shampooing hair, skin, and mouth/oral care and hygiene to include dentures) for assigned patients according to defined procedure.
  • Makes all types of beds and stretchers.
  • Assists with the use of bedpans and urinals. Applies external catheters and maintains patency or drainage.
  • Assists in providing ostomy care for established and uncomplicated ostomies.
  • Prepares patient for exam and treatment by taking & recording vital signs and other necessary measurements and recording in patient record
  • Documents pain scores as directed or reports to RN and/or LPN
  • Turns patients as required using all types of supporting and comfort devices. Assists or provides patient transfers, activities, e.g., ambulation, litter, and or wheelchair to bed.
  • Escorts patients to tests/procedures as needed.
  • Assists in diagnostic examinations by positioning, draping, the patient and passing instruments and supplies.
  • Takes and records temperatures, blood pressure, pulse, O2 saturation, pain, height, and weight.
  • Collects and labels any urine, blood, stool, sputum, nasal swabs, nasal washing, and rectal swabs in the presence of the patient using two identifiers as per policy and delivers to the lab.
  • Applies cold/heat packs locally as ordered; Applies and removes binders and bandages as ordered.
  • Reports any changes in patient status to RN/LPN or Physician.

CREDITABLE EXPERIENCE:
(1) Knowledge of Current Practice. To be creditable, the experience must have required the use of knowledge, skills, and abilities associated with the current practice.
(2) Quality of Experience. To be creditable, NA experience must be documented on the application or resume and verified in an employment reference, or through other independent means.
(3) Part-Time Experience. Part-time experience as a NA is creditable according to its relationship to the full-time workweek. For example, a NA employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.

GRADE DETERMINATION:

GS-5 NA (Full Performance Level) Experience. One year of progressively responsible assignments and experience equivalent to the GS-4 level which demonstrates knowledge, skills, and abilities that are directly related to the specific assignment. In addition, the candidate must demonstrate the professional KSAs in subparagraph (c) below;

OR

Education. Successful completion of a 4-year course of study above high school leading to a bachelor’s degree that included 24 semester hours of courses related to health care or possession of a bachelor’s degree;

AND

Demonstrated Knowledge, Skills, and Abilities (KSAs)

  1. Ability to assist in the full range of nursing care to patients/residents with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse;
  2. Ability to communicate orally with patients/residents, families, interdisciplinary team and other personnel. This includes serving as a preceptor to new NAs by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting;
  3. Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. For example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis, etc.

Physical Requirements: The nursing assistant within all services may require shift work and or rotation of tour assignments. Routinely requires regular and recurring bending, stooping, walking, stretching, lifting, standing, and repositioning and transportation of patients or similar activities. Work requires the ability to physically intervene with disruptive patients and protect self and others from injury using the knowledge gained from ongoing training. This position will require the ability to work outside daily for duration of shift. Will need to follow facility guidelines for COVID-19 vaccination and influenza vaccination of employees.
